Day of Procedure
When you arrive at the Center we ask that you check-in at the registration desk to verify personal and insurance information and provide us with a means to contact your ride home upon discharge.
- Please anticipate being at the center for two to three hours on your procedural day.
- if you are receiving anesthesia you will need to arrange for a responsible adult to bring you to the surgery center and to drive you home.
- Bring your insurance card, payment obligation, and driver’s license or photo I.D. with you. Leave all valuables at home as we can not be responsible for damaged or lost property.
- Be prepared to sign a form giving your consent for the procedure. If the patient is under 18, a parent or legal guardian must accompany the patient and sign the consent form. If the patient is not able to sign legal documents, then someone with a Durable Power of Attorney should be present. The surgery center does not honor Advanced Directives. For additional information regarding a Power of Attorney or Advanced Directives, contact the surgery center prior to your procedure.
